You can either have an initial consultation ($35) and purchase your products a la carte (sunscreen required) or, choose an all-inclusive customized package below that includes an array of full-size professional products and future phone re-evaluations.

Advanced Acne Package

Fill out your Intake Questionnaire and Skin Therapy Agreement completely and submit via email. Or, print out and fill out in dark ink and fax these forms to 510.834.8153.
Be very specific when you describe your skin problems. This will help us address each of your concerns and let us know what treatments and active ingredients you’ve used on your skin in the past. We’ll discuss any known allergies, sensitivities, medications you’re currently using, etc. We’ll also address the many lifestyle issues that can affect your skin.
Your first telephone consultation will take 20 to 30 minutes to address your skin and lifestyle and review the specialized products you’ll be using. We’ll make every effort to accommodate your needs, schedule and product preferences.
Long Distance Packages include initial phone consultation, full-size home care products, detailed instructions, unlimited email support, telephone follow-up evaluations and written information about your skin or scalp disorder.
Shipping Policy: We ship via UPS Ground, Overnight, Two-Day Air, Three-Day Ground and Federal Express. We ship via US Postal Service for overseas and military only. Shipping/handling and applicable sales tax (California residents only) is additional. We ship to your credit card billing address. Rush service and express shipping is available upon request and will be subject to increased shipping fees.
Minimum for US Orders: $50, excluding sales tax (California residents only) and shipping/handling.
Minimum for International Orders: $100 USD, excluding international shipping/handling.
Shipping and Handling: These fees are additional and based on weight, final destination, distance and shipping method. For security reasons, we ship to the credit card billing address only and your signature is required. Once your package leaves the US, Urban Skin Solutions, Inc. is not responsible for, and has no control over, (a) international customs delays, (b) import duty, or (c) customs brokerage fees charged to you .
Payment: We accept major credit cards, debit cards, PayPal, money orders, travelers’ checks and personal checks with valid photo ID. Credit cards must be verified and orders must be shipped to the address on your credit card account and signed. All shipments require a signature upon delivery. We make every attempt to ship your order within two business days, but please understand that we can get very busy. Orders placed after 12 noon Pacific time on Friday might not ship until the following Tuesday afternoon. We are unable to ship on Saturday, Sunday or Monday. Please plan ahead and anticipate your needs. Always allow three days to process and pack your order and up to eight working days in transit, depending on where you live and how your order is shipped.

Personal Checks must be deposited and clear our bank before we can ship. Please anticipate your needs and give yourself plenty of time. Allow up to two weeks for delivery. Returned checks are subject to a $25 processing fee.
Guarantees, Refunds and Exchange Policy: Urban Skin Solutions cannot give promises, time limits or guarantees. Skin care involves genetics (skin conditions often run in families), stress, hormones, diet, water intake, medical conditions, obesity, medications, shaving/haircut habits, sun exposure, diligent sunscreen use, consistent home care compliance, regular follow-up evaluation, professional treatments and a variety of lifestyle issues. Since we can’t “go home with you” to monitor your life, we are unable to give a “Money Back Guarantee” on our packages, products or treatments.
Product Use: Use products sparingly, exactly as directed, and massage into the skin until product is fully absorbed. To avoid dryness and irritation, do not apply too generously or too often. Apply and reapply sunscreen as directed, even on overcast days. Contact us immediately if you have questions or concerns.
Returns and Exchanges: Cosmetic and active ingredients occasionally cause sensitivity or an allergic reaction, which usually occurs within the first three days. In the event of a true allergic reaction, manufacturer defect or problem with a specific product, stop using that product. Contact the office immediately to schedule a brief visit or phone call so we can follow-up with you, discuss the problem and if needed, arrange to exchange the problem product. With prior written authorization, a problem product can be returned and exchanged within the first ten days of purchase. No cash refunds. No product exchanges or returns after ten days.
Product Refills are not included in program cost and can be purchased individually without an appointment. However, clients who use our clinical-strength products are required to purchase appropriate sun protection every 90 days or sooner, and to apply and reapply an adequate amount on all sun-exposed skin, exactly as directed.
Intake Forms: All current clients are required to complete new intake forms (so we can update your file) on a yearly basis. Mailorder Packages available for out-of-town and relocated clients only and require a telephone consultation.
Privacy Policy: Urban Skin Solutions never shares its client mailing list or information with third parties for any reason.
