Series of Six Treatments for the Prepaid Price of Five – Prices vary; restrictions apply

Important Notice: Completed consent form required prior to service. Extras and add-on services will be charged at time of service. Rewards Vouchers are valid toward basic peel services only, expire after 90 days from date of issue and are non-transferable to others. No exceptions. Treatments included with Clinic Programs valid for 90 days. Series of Six treatments valid for six months from date of purchase. Late cancellations or no-shows for treatments included in prepaid packages will be voided.

A $25 fee will be charged for missed or canceled appointments with less than a 24-hour notice for weekday appts. or 48-hour notice for Saturday appts.

Chronic no-shows and those who cancel without notice, appointments must be prepaid or walk-in.

No infants or unattended children. No guests, babies or children in treatment rooms.
